Dr. Gamboa is Founder & CEO of Body Dynamics Center for Sustainable Health. She is a Doctor of Physical Therapy and a Board-Certified Orthopedic Clinical Specialist. Jennifer is recognized as a leader in the field of orthopedic and performing arts physical therapy and a pioneer in the development of an integrated health-focused practice for physical therapy, health promotion, and primary prevention. She teaches nationally and internationally in performing arts medicine, integrated practice models, and exercise prescription for chronic disease, and is working with the University of Montana to develop a Lifestyle Intervention Certification Program for PTs and allied health promotion providers.
Jennifer continues to work actively with clients needing both rehabilitation and fitness programming and to mentor other therapists in the field. She serves on the Board of Directors of the Arlington Free Clinic. Jennifer and her husband Mario have two children, Isaac and Sophie, and spend many hours supporting their soccer, gymnastics, basketball, and track activities. In her spare time, she likes to read, garden, ski, and be outdoors.
Jerry Durham is Co-Founder and Chief Relationship Officer at San Francisco Sport and Spine Physical Therapy. He has been a business owner for over 15 years now. As the founding owner of Excel Physical Therapy, David strives for his clinics to deliver unprecedented excellence with patient care in the Gallatin Valley. David established Excel in 2001 on the principles of specialization, advanced education and customer service.
David specializes in the treatment of back and neck pain, spinal issues, whiplash, headaches, TMJ/jaw pain, and postural dysfunctions. David received his masters in physical therapy from Mount St. Mary’s College in Los Angeles, California. He enjoys finding long-term solutions for his patients — solutions that involve a fine-tuned combination of manual manipulative therapy and a targeted exercise program that address even the most difficult patient presentations.
A considerable amount of David’s advanced training occurred through the North American Institute of Orthopedic Manual Therapy (NAIOMT). He has completed advanced certification in manual therapy (CMPT) with NAIOMT, and he has received advanced training in dry needling techniques for the spine and extremities. David is a Certified Clinical BikeFit Pro Fitter.
David enjoys spending time with his family, cooking, Nordic skiing, road biking, fly-fishing and hunting.
A Clarence resident, Hughes is a graduate of Ithaca College’s Physical Therapy program and holds a master’s degree in Health Service Administration from D’Youville College.
Most recently, she owned and operated SportsPlus Physical Therapy, PC, a private physical therapy practice in Batavia focusing on general orthopedic and sports injury rehabilitation. Prior to that, she worked for two Catholic Health organizations, McAuley-Seton Home Care and AthletiCare at Kenmore Mercy Hospital, as a physical therapist and certified athletic trainer.
As director of outpatient rehabilitation services for Catholic Health Continuing Care, Hughes had direct oversight for operations, quality assurance, business development and management of outpatient rehabilitation and sports outreach services throughout Catholic Health.
Mike Eisenhart is a leader in the physical therapy profession having held posts at the local, state and national levels for the American Physical Therapy association, the nation’s largest professional association for physical therapists including the role of President of the association’s NJ chapter from 2015 to 2017. Mike is also the Managing Partner of Pro-Activity in Lebanon, NJ.
In addition, as one of the profession’s top innovators, formally recognized in 2013 for work done on Pro-Activity’s Prevention Model, which he pioneered initially in 1999, Mike has written extensively ranging from blogs and trade journals to more formal peer-reviewed publications. His most recent formal work was the chapter “Work Injury Prevention and Management: The Role of the Physical Therapist in Injury Reduction/Prevention and Workforce Wellness” in 2014, on behalf of the national Orthopedic Section of the APTA.
Professionally, Mike is perhaps most widely known as a passionate speaker/presenter and coach, having lectured on health and prevention to thousands of clients, employers, professionals and students, in audiences both small and large over the years…..and when he’s not chasing his kids or logging the swim-bike-run miles, he can be found….sleeping of course.
Brad Deal serves as Rehab Director at Stewart Physical Therapy Clinic in Burlington, North Carolina and oversees the operation of eight outpatient facilities including 21 therapists and assistants. Mr. Deal has been a practicing physical therapist for 25 years as well as a Certified Orthotist since 2003.
Prior to joining Stewart Physical Therapy in 2002, he served as Western NC Regional Manager for Rehability Corporation and which included overseeing the daily operation of 10 outpatient clinics throughout the Piedmont and Western portions of NC. He serves as a Member of Clinical Advisory Group of Clinicient, Inc. He graduated from the University of North Carolina at Chapel Hill in 1990 with a BS in Physical Therapy.
Michelle is the CEO of a multi-clinic physical therapy practice in Rhode Island. Originally from New Zealand, Michelle came to the USA in 1996 for an outpatient hospital position and transitioned to supervisor of the Physical Therapy Department. In 2001 she began working at a small private practice, Performance Physical Therapy, and in 2003 Michelle purchased Performance from the founder. Michelle has lead Performance through significant growth, to a practice with over 100 employees and 9 locations. Performance has received much recognition; in 2012 the Outstanding Philanthropic Business Award from the Association on Fundraising Professionals, in 2014 and 2015 ‘Best Places to Work’ and in 2014 the Jane L. Snyder Private Practice of the Year award from the APTA.
In 2005 Michelle received her advanced masters and doctorate from MGH-IHP in Boston and was awarded the Ionta award for Clinical excellence. In 2006 she became a Board Certified Orthopedic Clinical Specialist and was chosen for the RIAPTA Distinguished Service award. In 2010 Michelle was a Providence Business News, 40 under forty honoree and in 2012 received the Corporate Heroes Award for her philanthropic work with Reach Out and Read, RI, where she now serves as the chair of the Board of Trustees. In 2014 she was named a 2014 Entrepreneurial Women to Watch Awardee from the RI Small Business Journal.
Michelle is the president of the RIAPTA Private Practice Group and serves as the chair for the National PR and Marketing committee for private practice physical therapy.
In addition to her role as a CEO, Michelle is a runner, snowboarder, traveler, wife and the mother of two children.
John founded Physical Therapy & Sports Rehab, based in Norwood, Massachusetts, in 1983 and co-founded the New England Physical Therapy Network, the largest network of independent physical therapy clinics in Massachusetts and New England. John has pioneered new approaches to physical therapy, developing and trademarking a number of formalized programs, including The Best Therapy Experience®, which was intended to address the lack of cohesive treatment approaches across practices and therapists for the consumer. He also created The Client/Therapist Game Plan™ and The Express Treatment Package™. Based on his proven platform of the same name, John authored The Best Therapy Experience®, published in 2014, which outlines the challenges the industry faces today and provides therapists with tested concepts and methods that are focused on the clients’ expectations. Through his book, work and advocacy, John continues to pursue his goal of helping to create efficient and meaningful solutions for the physical therapy profession, physical therapists and the consumer.
John received his B.S. in Physical Therapy from Northeastern University in 1978.
Donn originally hails from the Chicago area and is currently the Executive Director of Clinical Operations of The KOR Physical Therapy in Portland, OR. His practice encompasses many levels of clients, from high level athletes to the general active population. Donn has worked with many baseball players including Mike Stutes (Phillies), Chris Archer (Rays), and Joey Mahalic (Indians). Donn has also worked with various Chinese Olympic Teams to help decrease their incidence of injury. Donn has authored two books; The Bare Minimum: Baseball, Essential Training for the Baseball Athlete (available on Amazon.com) and The Bare Minimum: Donatelli Shoulder Method. He is a contributing author to Dr. Donatelli’s Sports Specific Rehabilitation, Orthopedic Physical Therapy, and Physical Therapy of the Shoulder, 5th ed. Donn lectures nationally to other physical therapists on how to assess and treat patients with shoulder injuries.
Donn enjoys biking, running, resistance training, watching the Chicago Bears, and spending time with his wife and son.
John Woolf is the Owner and former CEO of ProActive Physical Therapy in Tucson, Arizona. He also serves as a Member of Clinical Advisory Group at Clinicient, Inc. He is the co-director of the International Academy of Orthopedic Medicine-US and was Director of Sports Medicine for The University of Arizona, where he coordinated rehabilitation and medical coverage for division I athletes.
He belongs to the American Physical Therapy Association and the National Athletic Trainers’ Association and has lectured in the University of Arizona’s Sports Medicine Fellowship Program and continues to lecture nationally on the topics of diagnosis and management of musculoskeletal pain and the application of a biopsychosocial approach to health care. A graduate of the Northern Arizona University College of Physical Therapy, John has a masters degree in Exercise and Sports Science and Athletic Trainer Certification from the University of Arizona.
